Referral form template

This is for hiring managers, recruiters and team leaders who want to create (or simplify) their employee referral program.

Totally up to you. In our referral form template above, we’ve included a few key questions: name, contact details, position and a little room to explain why they’d be a great fit. But you can dig as deep as you like—you could even ask them to attach a CV to speed up the screening process.

2. Step two, connect Typeform with Google Sheets or Trello—whichever makes sense for you.
3. Then once you’ve shared the referral form with your team, watch the referrals roll in.
