At least annually, and after significant changes to systems, staff, or business operations. High-risk industries should review quarterly. Continuous monitoring tools supplement but don't replace periodic manual review.
It covers core controls aligned with frameworks like the NIST Cybersecurity Framework and CIS Controls. For specific regulatory requirements (HIPAA, PCI-DSS, SOC 2), a compliance-specific assessment is needed alongside this general review.
IT and security staff complete the technical sections. Business leadership should review risk areas that involve governance decisions. For small organizations without dedicated IT, an external consultant is worth the investment.
Multi-factor authentication. Enabling MFA across all systems — especially email and remote access — prevents a significant percentage of account compromises at minimal cost. If your organization does nothing else, do this.
Prioritize gaps by risk severity (likelihood Ă— impact). Create a remediation plan with owners and timelines. Review progress at the next assessment. Small, consistent progress beats ambitious plans that never get executed.
