When an incident happens at a childcare center — a fall, an altercation, a near-miss — accurate documentation is essential. This form gives staff a structured way to record what happened while memories are fresh, ensuring nothing important is omitted.
The template covers the child's name and age, the date, time, and location of the incident, a description of what happened, the witnesses present, the injury or outcome, the first aid or intervention provided, how the parent was notified and what they were told, and the staff member's signature.
A complete incident report protects the center, informs families, and creates a record for tracking patterns that might indicate a recurring safety issue.
As soon as possible after the incident — ideally within the same shift. Details fade quickly, and a prompt report is more accurate and more credible if the record is ever reviewed.
Parents should be notified the same day for any injury or significant incident. For minor incidents, a written report at pickup is standard. For medical emergencies, parents should be called immediately.
Any injury, altercation between children, near-miss that could have caused injury, missing child situation, medication error, or exposure to a potentially dangerous substance. When in doubt, document it.
Most licensing regulations require incident reports to be retained for at least 3 years. For incidents involving injury or complaints, retain longer. Check your state's childcare licensing requirements.
Serious incidents — significant injuries, hospitalizations, or situations involving abuse or neglect — must typically be reported to the licensing authority or child protective services. Know your mandatory reporting obligations before an incident occurs.
