Filing a complaint shouldn't be another burden on top of whatever incident prompted it. But when the process requires visiting a station in person, waiting in line, and filling out a paper form under fluorescent lights, many people give up or delay reporting. That means incidents go undocumented, patterns go unnoticed, and community trust erodes.

This police complaint form template provides an accessible, digital alternative for non-emergency reporting. It walks complainants through the details. What happened, when, where, who was involved, and what evidence is available — in a structured, one-question-at-a-time format that's less intimidating than a blank incident report. Conditional logic adjusts follow-up questions based on the type of complaint, ensuring relevant details are captured without overwhelming the person filing.

Customize it with your jurisdiction's categories, required disclosures, and follow-up procedures. Embed it on your department's website and connect it to your records management system through integrations so every complaint is logged, tracked, and routed to the right division.

Police Complaint Form Template FAQs:

A police complaint form is a document that citizens use to formally report incidents, concerns, or grievances to a law enforcement agency. It can cover a range of issues — property crime, noise complaints, suspicious activity, officer misconduct, or civil disputes. The form creates an official record that the department can use to investigate, track trends, and follow up with the complainant.

This form is designed for non-emergency reports — incidents where there is no immediate threat to life or property. Examples include theft (after the fact), vandalism, noise disturbances, harassment, fraud, property disputes, and complaints about officer conduct. The form should clearly state that emergencies require calling the emergency number (911 in the US) and that this form is not monitored in real time.

  • Complainant's full name, address, and contact information
  • Date, time, and location of the incident
  • Type of incident or complaint category
  • Detailed description of what occurred
  • Information about suspects or involved parties (if known)
  • Supporting evidence uploads (photos, videos, documents)

Include an option to submit anonymously, but note that anonymous submissions may limit the department's ability to investigate or follow up. Use conditional logic so that when "anonymous" is selected, the personal information fields are skipped, but the incident details section remains comprehensive. Some jurisdictions require complainant identification for certain types of reports. Build those requirements into the form logic.

Clearly communicate the process on the form's thank-you screen. Include an estimated response timeline, a case reference number (generated automatically if possible), and contact information for follow-up inquiries. Set expectations. Not every complaint leads to an investigation, but every submission is logged and reviewed. Transparency about the process builds trust and encourages future reporting.
