The form creates an official record that the department can use to investigate, track trends, and follow up with the complainant.

This form is designed for non-emergency reports — incidents where there is no immediate threat to life or property. Examples include theft (after the fact), vandalism, noise disturbances, harassment, fraud, property disputes, and complaints about officer conduct. The form should clearly state that emergencies require calling the emergency number (911 in the US) and that this form is not monitored in real time.

Some jurisdictions require complainant identification for certain types of reports. Build those requirements into the form logic.

Clearly communicate the process on the form's thank-you screen. Include an estimated response timeline, a case reference number (generated automatically if possible), and contact information for follow-up inquiries. Set expectations. Not every complaint leads to an investigation, but every submission is logged and reviewed. Transparency about the process builds trust and encourages future reporting.
