This template collects the reviewer's name and email, an overall rating, ratings on specific dimensions (quality, value, packaging, delivery speed, and so on depending on your product), a headline, the review body, and a photo upload for visual products. Optional fields for how the reviewer found you and what they would improve round out the qualitative picture.
Trigger it automatically via email a set number of days after delivery so the request is timely. Responses can populate your site's review section directly or sync to a review management platform. Either way, you own the data and aren't dependent on third-party review policies.

For reviews displayed on your own site, a small incentive (discount, loyalty points) is generally fine as long as you're transparent about it and don't make positive reviews a condition of receiving it. On third-party review platforms like Google or Amazon, incentivized reviews violate terms of service.
Connect the review request trigger to your order management system so requests are only sent to confirmed buyers. In the form, include the order number as a pre-filled field that links the review to a specific transaction. This prevents incentivized fake reviews and gives reviews a 'verified purchase' credibility signal.
