Follow-up survey template

See how customers feel once the first impression has worn off, so you identify the problems that only show up later. Send a timed check-in that measures satisfaction, resolution, and what's changed since you last heard from them.

It's easy to measure how someone feels right after a purchase or a support call, when the interaction is fresh and the goodwill is high. But that first reaction doesn't tell you whether the product held up, the fix actually stuck, or the customer quietly soured a month later. Typeform's follow-up survey checks back in after that first moment has passed, when the real verdict has set in.

A follow-up survey asks how things stand now—whether the issue stayed resolved, how the product's working out, and whether the customer would still recommend you. A customer who says the problem came back gets routed to a support follow-up, while one who's happy is asked to leave a review.

Over time, the follow-up responses show which fixes actually last and which customers are drifting before they churn. You catch the issues that a one-time survey misses, and you reach the customer while there's still a chance to turn things around.

Follow-up survey template FAQs:

A follow-up survey is a short questionnaire sent in the days or weeks after an interaction, purchase, or support ticket to check how things stand now. It captures whether an issue remains resolved, how a product or service is holding up, and how the customer feels once the first impression has faded. Support and customer success teams use follow-up surveys to catch the problems that surface later, not just the reaction in the moment.

Without a follow-up survey, you only ever see the customer reaction at the moment of the interaction, when goodwill is high and additional problems haven't surfaced yet. A fix that fails a week later or a product that disappoints on day 30 is feedback that never makes it back to you, and the customer churns without a word. A follow-up survey reaches them after the fact, so a problem that developed over time still reaches you while you can act on it.

Capture how things stand now:

  • Whether the original issue is fully resolved
  • How the product or service is working out
  • Any new problems since the last contact
  • Overall satisfaction now versus then
  • Whether they'd recommend you

Send a follow-up survey once enough time has passed for the real experience to show—often a week or two after a support ticket closes, and closer to a month after a purchase. Send it too soon and the responses just mirror the on-the-spot reaction; too late and the details fade or the customer's already gone. Match the survey timing to how long your product or fix takes to prove itself.

Yes. You can use conditional logic to send each respondent down the right path, so a customer reporting a recurring problem is handed straight to support while a satisfied one is invited to leave a review. Their answers stay tied to the original interaction, so whoever follows up has the full context instead of starting cold.

Travel Agency Feedback Form

Travel Agency Feedback Form

A generic satisfaction survey emailed three weeks after a trip isn't the kind of feedback an agency can act on, especially once the specifics of a rough hotel transfer have faded. Collect feedback on the specific parts of a trip while it's still fresh, so a complaint that needs immediate follow-up doesn't sit behind a stack of routine responses.

Medical Teaching Feedback Form

An evaluation handed out on the last day of a rotation, filled out in the hallway before everyone scatters, isn't the kind of feedback that helps a program improve teaching quality. Collect specific, timely feedback on teaching encounters before it fades into a generic "good rotation" comment.
