Immediate feedback tells you what participants thought during the program. Follow-up feedback tells you what actually stuck. This version of the feedback form is sent 30 to 90 days after program completion to assess whether skills were applied, goals were achieved, and impact was sustained.
The template covers whether the participant has applied what they learned, specific examples of application, the degree of change in relevant outcomes (skills, knowledge, behavior, results), any barriers to applying the program content, what they wish they'd learned or practiced more, and their current overall assessment of the program's value.
Longitudinal feedback data is the strongest evidence of program impact — and the most compelling data for funders, sponsors, or leadership asking whether the program is worth continuing.
30 days for behavior change assessment, 60 to 90 days for skill application and outcome tracking. For longer-term programs, a 6-month follow-up can assess sustained impact.
Collect email at program registration and communicate the follow-up plan clearly at the end of the program. Participants who expect a follow-up survey are more likely to complete it.
Follow-up surveys typically see 20 to 40% response rates — lower than immediate post-program surveys. Personalized outreach from a specific staff member improves response rates significantly over automated bulk emails.
