A student attendance form records whether a student was present, absent, or tardy on a given day, along with whether an absence was excused, and any documentation supporting that. It's the underlying record that daily attendance tracking, truancy determinations, and reporting all depend on.
It depends on what's being shared. FERPA treats "dates of attendance," the general period someone attended, like a school year, as directory information a school can disclose without consent. But that term specifically excludes daily attendance records, whether a student was present or absent on a particular day, which remain protected and generally require consent to disclose beyond school officials with a legitimate educational interest.
It varies by state, and there's no single national threshold. Some states set it at three to five unexcused absences in a school year, others count three consecutive unexcused days, and some measure chronic absenteeism as missing 10 percent or more of the school year for any reason. Confirm your specific state's definition rather than assuming a standard number applies everywhere.
Consequences can reach both the student and the parents. Parents can face fines that commonly start at $25 to $100 for a first offense but can reach $500 to $1,000 or more in some states, plus court fees and, in severe cases, jail time. Students can face consequences including delayed or suspended driving privileges, community service, or court-ordered monitoring. Courts generally treat parents who engage cooperatively far more leniently than those who ignore notices.
