The template covers the client's name, age and ID verification, health disclosures relevant to tattooing (blood thinners, skin conditions, diabetes, pregnancy), the tattoo design and placement description, aftercare acknowledgment, consent to the procedure, and a liability waiver with dated signature.
A client who arrives having read the form is better prepared for the session and aftercare. A signed form means both parties have documented the agreement — including the client's confirmation that they're proceeding voluntarily.
