A leave request process that relies on verbal conversations or informal emails creates confusion about approvals, balances, and coverage. This form gives employees a simple way to submit their request and gives managers a consistent record to work from.
The template captures the employee's name and department, the type of leave being requested, the start and end dates, the reason for the request, and a note about coverage arrangements if applicable. You can add a manager notification step that routes the request automatically.
Because every request goes through the same form, your HR records stay organized. You can track request history, filter by leave type or department, and spot coverage gaps before they become problems.
