Equipment Release Form
Release equipment without a paper sign-out sheet that's missing by the time gear comes back. Log who took what, its condition, and their signature before it leaves the building.
Equipment usually leaves on a clipboard or a text to whoever's at the front desk. Condition gets noted from memory, nobody signs anything, and if a tool comes back scratched or a laptop doesn't come back at all, there's no record of who had it last or what state it was in.
Typeform's equipment release form captures that at the moment gear goes out, who's taking it, what it is, and its condition, before anyone walks out the door. Conditional logic adjusts the questions to what's leaving: a vehicle asks for mileage and fuel level, a tool or electronic item asks for a serial number and accessories included. Add a Signature question so whoever's taking equipment signs off on the spot, typed, drawn, or uploaded, and attach photos of its condition with File Upload.
Every release comes dated, itemized, signed, and photographed, so if something comes back damaged or doesn't come back at all, you're not relying on someone's memory of what happened. You know exactly who had it, when, and in what shape.
An equipment release form is an online form that documents equipment being checked out or handed over to someone, an employee, contractor, or renter. This includes who's taking it, what it is, its condition at release, and when it's due back. It creates a record of the handoff instead of relying on a verbal agreement or a sign-out sheet.
A paper sign-out sheet gets filled in inconsistently, sits at a desk where it's easy to skip, and doesn't usually capture conditions or a signature. An equipment release form asks the same questions every time and can require a signature before the submission counts as complete, so you're not left guessing who took what and in what shape it left in.
Capture what you'd need if the equipment doesn't come back the way it left:
- Name and contact details of who's taking the equipment
- Equipment description, including serial or asset number
- Condition at release, with photos if possible
- Date released and expected return date
- Signature confirming the equipment and its condition
Not necessarily. For high-value equipment or situations with real legal exposure, you may still need a separate rental, lease, or liability agreement that spells out who's responsible if something's damaged or not returned. Use this form to document the release itself since it doesn't substitute for that agreement, so confirm what your equipment or legal policy requires before you rely on it alone.
Yes. Typeform connects natively to Google Sheets and Airtable, so every release logs to a running sheet automatically. There’s no exporting responses or re-typing them into an asset tracker. Set it up once and each new release appears as a row the moment someone signs.
