Informed consent isn't just a signature at the bottom of a page. Document exactly what was disclosed before a procedure, not after something goes wrong and the file gets pulled.
A signature on a consent form doesn't tell you much on its own. It doesn't say whether the patient was walked through and explained the risks, the alternatives, or their right to refuse, or whether they had the capacity to consent at all. If a complication happens and the file gets pulled, a blank signature line is a bigger problem than the complication itself.
Consent also isn't valid if the patient couldn't actually understand it. A minor generally needs a parent or guardian to sign instead, and a patient who wasn't given the form in a language they understand hasn't meaningfully agreed to anything, regardless of what's on the signature line. Skipping either of those doesn't just weaken the documentation, it can mean consent never legally happened at all.

- Patient's (or guardian's) signature and date
Not exactly. A signature only reflects what was actually disclosed, if a required element was skipped or the patient lacked capacity to consent, the form may not hold up as valid consent. Where there's real doubt, that's a question for legal counsel, not something a form template settles on its own.
