If this questionnaire is part of a research study, and not just an internal survey, there's a step that has to happen before the first response comes in, not after. One well-known exception: Pfizer made the PHQ-9 and GAD-7 freely available without copyright restriction in 2010 specifically to encourage broader use, but not every instrument gets that treatment, so confirm the terms for whatever scale you're borrowing from.
