Whether you run a restaurant, a catering company, a corporate cafeteria, or a meal delivery service, feedback is the difference between repeating mistakes and improving your menu. But in-person feedback is unreliable. People are polite to servers and honest on forms. And without structured data, you're making menu decisions based on which complaints were loudest.

This food survey template captures honest, actionable feedback on food quality, variety, presentation, portion size, dietary options, and overall dining experience. Conditional logic adjusts the survey based on the dining context. A restaurant survey asks about service speed, while a meal kit survey focuses on recipe clarity and ingredient quality.

Share it via receipt QR codes, post-meal emails, or table tent cards. Responses flow into a dashboard where you can spot trends, track satisfaction over time, and make menu decisions backed by data.

Food Survey Template FAQs:

A food survey is a structured questionnaire that collects feedback on food quality, taste, presentation, variety, and the overall dining or eating experience. Restaurants, caterers, food services, and meal delivery companies use them to understand customer preferences, identify improvement areas, and make data-driven decisions about menus and operations.

Because taste is subjective, and your assumptions about what's working might not match reality. A dish you think is a hit might be ordered out of limited options, not preference. A food survey reveals what customers love, tolerate, and avoid. Giving you the insight to double down on winners, rework underperformers, and introduce offerings people actually want.

  • Overall satisfaction with the food (rating scale)
  • Ratings for specific attributes: taste, freshness, presentation, portion size
  • Variety and dietary option satisfaction (vegetarian, vegan, gluten-free)
  • Favorite dish or menu item
  • Item they'd most like to see added or changed
  • Open-ended: "Any other feedback about your dining experience?"

Timing and incentive. Send the survey within 2 hours of the meal while the experience is fresh. Keep it under 2 minutes — 5-7 questions maximum. Offer a small incentive: a discount on the next order, entry into a monthly drawing, or a free side. QR codes on receipts or table tents work well for dine-in; automated post-order emails work for delivery and takeout.

Look for patterns, not individual comments. If 40% of respondents say portions are too small, that's actionable. If one person says the pasta is too salty, that's noise. Track satisfaction scores by dish over time to identify declining favorites. Cross-reference dietary option feedback with ordering data to see if there's unmet demand. The survey tells you what to investigate — your taste tests and trials tell you what to change.
