Waxing without a proper consultation is guessing. Medications like Accutane, retinoids, and blood thinners make skin significantly more vulnerable to waxing-related reactions. A client who doesn't mention they've just had a chemical peel or that they're on a new prescription leaves you managing a preventable outcome. A consultation form prevents that.
This template collects the client's contact details, current skincare routine and products used on the waxing area, medications and supplements, recent cosmetic procedures, known allergies and skin sensitivities, and what area they're coming in for. It asks directly about the contraindications that matter most — and asks them to confirm they haven't used retinoids or exfoliants in the days before their appointment.
Send it before the first appointment so you have time to review it and flag anything that requires a modified approach or a deferred service. For returning clients, a quick annual update captures changes in medications or skincare without repeating the full intake each time.
Isotretinoin (Accutane) — and similar retinoids — is the most significant contraindication; clients typically need to be off it for 6 months before waxing. Topical retinoids, AHAs, BHAs, and high-dose vitamin C also increase skin fragility. Blood thinners increase bruising risk. Ask about all of these on the form, by name.
At least 24 hours before the appointment — enough time for you to review it and contact the client if anything needs clarification before they come in. Same-day completion means you're reviewing it with the client in the room, under time pressure, rather than being able to think through the best approach.
Yes. A brief update at each visit — or a full re-intake annually — catches medication changes and new skincare products that have been introduced since the last appointment. A simple 'anything changed since your last visit?' question at the start of each service is a reasonable minimum.
Assess the severity and advise accordingly. For active Accutane use, decline the service. For topical retinoid use, ask when they last applied it and to the specific area — a recent application to a different area may be fine, while recent use on the treatment area is a clear reason to reschedule. Document your decision in the client's record.
Yes. The contraindications are largely the same across areas. For facial waxing near the brows, add questions about recent brow tinting, Botox, or facial treatments. For intimate waxing, add questions about current infections or skin conditions in the area. The base template covers the core — add specifics for the services you offer.
