Model scouting and agency applications work better when every applicant provides the same structured information. This form gives agencies a consistent way to evaluate applications and gives models a clear, professional process to present themselves.

The template covers the applicant's name and contact information, physical measurements (height, clothing sizes, shoe size), photo submissions (headshot, full-length, any portfolio work), prior modeling or performance experience, special skills or training, geographic availability and travel flexibility, and representation history.
