Always start with the basics. You’ll want the user’s name, their contact info, and which site permissions they’d like.

From there, it’s up to you. You can also add a field for their manager’s contact info, in case you need their approval. And if you’re on one of our Paid plans, you can even add a File Upload question to collect users’ profile pictures.
