A participant registration form for a research study isn't just an intake, it's often where informed consent has to happen too. Under the Common Rule (45 CFR part 46), research involving human subjects generally requires informed consent that covers the purpose of the study, foreseeable risks, expected benefits, and the fact that participation is voluntary and can be withdrawn at any time. A registration form that only asks for a name and an availability slot skips the disclosure a study is required to make before anyone actually participates.

Some populations need more than a standard consent flow. A minor generally can't consent for themselves, a parent or guardian has to, and studies involving certain vulnerable populations or sensitive topics often require extra review most registration forms don't build in. Data use adds another layer: a participant needs to know upfront whether their information will be shared, published, or reused later, not learn about it after they've already agreed to something else.
