Supervisors shape the day-to-day experience of everyone who reports to them. Upward feedback from employees is one of the most valuable tools for developing effective managers — when it's collected consistently and taken seriously.

The template covers the supervisor's communication style and clarity, how well they set expectations and provide feedback, their availability and support, recognition of contributions, handling of conflict and difficult situations, development support, and overall rating. An open-ended question asks what the supervisor should keep doing and what they should change.

Managers who receive honest feedback — and act on it — improve. Those who never hear how they're perceived often develop blind spots that affect retention and team performance.
