Employee Opinion Survey Form Template

Hear what your team actually thinks — and build a workplace worth staying at.

Employee surveys that ask fluffy questions get fluffy answers. This form asks the questions that actually reveal how your team feels about their work, their management, and the organization — and gets answers honest enough to act on.

The template covers job satisfaction and engagement, confidence in leadership, clarity of goals and expectations, quality of communication, growth and development opportunities, workload and wellbeing, sense of belonging and inclusion, and an open-ended 'what would make this a better place to work' question.

When employees see that their input is read and responded to — not just collected — participation rates stay high and trust in the process grows over time.

Employee Opinion Survey Form Template FAQs:

Communicate what you'll do with the results before the survey launches. Share past examples where survey data led to change. And follow through — when the survey closes, share findings and commitments within 4 to 6 weeks.

Anonymous surveys typically yield more honest responses, especially when asking about manager behavior or perceived fairness. If you want to track responses by department or role, Typeform lets you include optional demographic fields without requiring personally identifying information.

10 to 15 minutes is the sweet spot. Longer surveys have lower completion rates and participants rush through at the end. If you have more topics to cover, run focused pulse surveys throughout the year rather than one annual marathon.

Share aggregated results transparently — the good, the average, and the challenging. Avoid cherry-picking positive results. Presenting honestly, including the areas needing improvement, signals that you take the feedback seriously.

One comprehensive survey per year, supplemented by short pulse surveys (3 to 5 questions) quarterly. This gives you a full picture annually and timely signals in between.

Work From Home Request Form

A Slack message asking to work from home on Thursday isn't a request anyone can track, especially once the same employee has asked five different ways over a month. Collect the request type, dates or schedule, and manager approval in one place, so HR isn't reconstructing who's working where from a scattered inbox.

Return of Property Form

A property checklist stapled to an exit interview packet isn't a record anyone can rely on when a laptop turns up missing three weeks later. Collect itemized equipment details, condition at return, and a signed acknowledgment before an employee's last day, so IT isn't chasing down a laptop that already walked out the door.

Early Retirement Form

A retirement mentioned to a manager in conversation doesn't give HR anything to actually process, and if there's an incentive deadline attached, the date that conversation happened can matter more than anyone realized at the time. Collect what your HR team actually needs, employee information, requested date, and benefit elections, before a window closes.

Staff Leave Application Form

A vacation request approved with a thumbs-up emoji in Slack isn't the kind of record payroll can work from when it's time to reconcile someone's leave balance. Collect the leave type, dates, and manager approval in one place, so a team doesn't accidentally double-book someone's time off.

Disability Accommodation Request Form

A quiet word to a manager about needing a different desk setup isn't the kind of request that starts a documented process. Collect the specific accommodation needed and any supporting information before a decision gets made informally, so a request doesn't get lost between a hallway conversation and HR's desk.

Parental Leave Request Form

News shared with a manager over a coffee break doesn't give HR the dates it needs to actually process anything. Collect what your HR team needs, requested dates, leave type, and benefit continuation, while there's still time to plan around it.
