A consistent weekly check-in gives managers visibility into what their team is working on, how they're feeling, and what's getting in their way — without adding unnecessary meetings. This form takes 3 minutes to complete and delivers the signals that matter.

The template covers what the team member accomplished last week, what they're focused on this week, anything blocking their progress, how they'd rate their energy and workload on a scale, and a free-text field for anything else they want to flag. You can add a question about help needed or priorities for the week.

Responses aggregate in real time. Managers can review the team's check-ins each Monday morning, follow up on blockers, and spot patterns over time — all without scheduling another meeting.

Weekly Check-In Form Template FAQs:

3 to 5 minutes. Longer than that and completion rates drop. Focus on the two or three questions that give you the most useful signal — what's happening, what's blocked, and how the person is doing.

Not if you want to follow up on blockers. But you can add an optional anonymous comment field for issues team members aren't comfortable raising directly. This gives you a pulse on morale without exposing individuals.

Review every submission before your Monday standup or one-on-ones. Look for repeated blockers, workload red flags, and team members who consistently rate their energy low. These are the signals that warrant direct follow-up.

It's especially valuable for remote teams where casual check-ins don't happen naturally. The form replaces the impromptu desk conversation that office-based teams take for granted.

Review individually each week and look at trends monthly. If you're not acting on what the form surfaces, team members will stop completing it carefully. Close the loop by addressing blockers visibly.
