When kids travel without both parents, documentation isn't optional. It's what prevents a stressful situation at the border or the airport.

A child traveling with one parent, a grandparent, a school group, or any non-custodial adult needs proper authorization. Without a signed consent form, you risk being stopped at border crossings, denied boarding at airports, or questioned by authorities — situations that are frightening for children and infuriating for adults who thought they'd covered their bases.

This child travel consent form template collects all the information authorities may request: child's details, traveling companion information, both parents' consent, itinerary, emergency contacts, and medical authorization. Conditional logic adjusts the form based on travel type (domestic, international) and the child's relationship to the traveling adult.

Share the form between separated parents, send it to school trip organizers, or use it in your travel agency's booking process. The completed form produces a clear, printable document that satisfies border agents, airlines, and anyone else who asks, "Do you have permission to travel with this child?"

Child Travel Consent Form Template FAQs:

A child travel consent form is a legal document that authorizes a minor to travel with someone other than their legal parents or guardians. It includes the child's identifying information, details about the traveling companion, the non-traveling parent's notarized consent, travel itinerary, and emergency medical authorization. It demonstrates to authorities that the child's travel has been approved by all custodial parties.

International border agencies, airlines, and law enforcement are increasingly vigilant about child trafficking and parental abduction. A child traveling with only 1 parent, a relative, or a non-family member may be asked to prove authorization, and a verbal explanation won't suffice. A written, signed consent form (ideally notarized for international travel) prevents delays, questioning, and the possibility of being denied entry or boarding.

  • Child's full legal name, date of birth, and passport number
  • Names, contact information, and relationship to child of the traveling companion(s)
  • Non-traveling parent/guardian's full name, contact information, and consent statement
  • Travel dates, destinations, and flight/transportation details
  • Emergency medical authorization and insurance information
  • Notarization block (for international travel)

For international travel, notarization is strongly recommended and sometimes required. It adds a layer of legal verification that border agents take seriously. For domestic travel, notarization is generally not legally required but still advisable for custody situations or when the child is traveling with non-family members. Some countries (Brazil, Mexico, South Africa) have specific consent form requirements. Check the destination country's embassy website before travel.

This is a custody issue, not a form issue. If one parent won't consent and you share legal custody, you may need a court order authorizing travel. Consult a family law attorney in your jurisdiction. In the meantime, do not attempt to travel internationally without proper documentation. It can result in the child being detained at the border and may create serious legal consequences for the traveling parent.
